Confirmation of the Polish citizenship or its loss
A decision confirming the Polish citizenship or its loss is issued at the request of the person concerned or of an entity which has demonstrated its legal interest or an obligation to obtain such a decision – by a provincial governor competent in terms of the current or last place of residence of the person concerned in the Republic of Poland, and if there is no such basis – by the provincial governor of Mazowieckie Province. The proceedings in this case can also be instituted ex officio.
An application for confirmation of the Polish citizenship or its loss contains data of the person concerned and his or her ascendants up to the second degree, as well as information about any significant circumstances necessary to determine the factual and legal status. A person and an entity lodging an application for confirmation of the Polish citizenship or its loss are obliged to submit documents confirming data and information contained in the application, unless such documents cannot be obtained due to obstacles which are difficult to overcome. An application for confirmation of the Polish citizenship or its loss is lodged with a provincial governor. A person who resides outside the Republic of Poland can lodge such an application via a consul competent in terms of the person’s place of residence. The consul hands over the application with the documents enclosed, as well as other documents and information about the person concerned, to the provincial governor, without delay.
