Documents for confirming the possession of Polish citizenship

In order to confirm the possession of Polish citizenship, it is necessary to file a form to Mazowiecki Urząd Wojewódzki (Mazowieckie Province Governor's Office) together with all required documents, consisting primarily of acts confirming the identity and the citizenship of the applicant, and of his or her ancestors.

The persons applying for the confirmation of the citizenship within that type of procedure are most often children, grandchildren or great-grandchildren of Polish emigrants. In such case, it is necessary to present the documents concerning relevant predecessors (parents, grandparents), whi were born in Poland, present the history of their migration and prove the consanguinity. The documents of the ancestors confirming the information provided in the form should be issued by Polish authorities or institutions. Examples of such documents include, above all:

  • birth and baptism certificates,
  • marriage certificates
  • death certificates,
  • passport files,
  • ID cards,
  • military documents ,
  • excerpts from the National Census of Population,
  • excerpts from registers,
  • lists of electors,
  • lists of ship passengers
  • insurance documents,
  • excerpts from commercial registers,
  • notarial files,
  • police documents,
  • medical records,
  • school diplomas,
  • decisions on the confirmation of Polish citizenship.

Apart from the abovementioned documents proving the facts indidated in the form, every application must also have the following attachments: authenticated copy of the passport, birth certificate, marriage certificate, a short resume and the address of the person applying for the confirmation of Polish citizenship.
